On a 12" monitor this means that it may have pushed it slightly off the right side of the window, which makes horizontal scrolling necessary. You can resize the columns to make them fit on the window. http://www.toodledo.com/info/help.php#62 Also, we will be improving note editing and horizontal scrolling issues in a future update.

Namely 1) Up/Down arrows now scroll 2) Page up/Page down buttons now scroll 3) Home/End buttons now scroll 4) Mouse wheel will now scroll even if the cursor is not inside the task area 5) We have fixed some spacing and alignment issues in the task section

If the alarm goes off some of the time and not others, it is not one of the obvious things. If I had to take a guess, I would say that the sound did go off, but another sounds from some other app maybe went off at the same time or shortly after that canceled the 5 minute ring tone from happening. You might try changing the ringtone back to a default sound and set some test alarms and see if that makes any difference.
